How admission actually works

• Register on Universitaly. Registration for the IMAT opens in late summer and closes well before the exam. Miss it and there is no second window that year.
• Sit the IMAT. A single multiple-choice paper covering reading and general knowledge, logical reasoning, biology, chemistry and physics/maths. India has just two test centres — New Delhi and Chennai — so most candidates travel to sit it, and seats at each are limited.
• Rank against the non-EU pool. Non-EU seats are ring-fenced and counted separately from EU seats, and each university publishes its own allocation. Your score is compared against other non-EU candidates for that university, not against the whole field.
• Accept your seat and enrol. Scrolling rounds reassign unclaimed places, so a score below the first published cut-off is not automatically a rejection.
• Legalise documents and apply for the visa. This runs in parallel and is where most delays occur.

What it costs, honestly

Italian public university tuition is assessed against family income rather than charged at a flat rate, so quoted ranges are close to meaningless without your own figures. What matters in practice is that a low-income Indian family frequently lands near the bottom of the scale, and a DSU scholarship holder can end up paying nothing at all while also receiving a grant toward accommodation and meals.

Living costs are the real variable. Milan and Rome are expensive; Pavia, Bari, Messina and Catania are markedly cheaper. Choosing a university is therefore partly a financial decision, not only an academic one — and the DSU scheme differs by region, which compounds the effect.

Coming back to India afterwards

An Italian medical degree from a public university is EU-recognised and lets you pursue specialisation and practice across Europe. If you intend to return to India, you must meet National Medical Commission requirements — including the screening examination — exactly as graduates of any other foreign medical school do. Check current NMC criteria, including course-duration and internship rules, before you commit; they have changed more than once in recent years.
